Job summary

Ruby's Place dba Restorative Pathways is seeking a Clinical Case Manager in Tracy, CA for an in-person, full-time role. You will provide integrated case management and clinical support to adults experiencing homelessness, bridging medical care, behavioral health, and housing services.

You will conduct psychosocial assessments, develop care plans, coordinate with medical providers, and ensure trauma-informed, survivor-centered services while meeting program targets and documentation requirements.

Qualifications

• Master’s degree in case management or MA/MS in counseling, or related field.
• Experience in crisis intervention, case management with victims of violence.
• Training in trauma-informed care, overdose prevention, and crisis intervention.
• Experience in domestic violence, human trafficking, or homeless settings.
• Understanding trauma-informed care and serving diverse populations.
• Flexible, proactive, and eager to learn.
• Knowledge of community resources, social services, housing programs.
• CA 40-hour Domestic Violence and Human Trafficking counselor certification (to be scheduled after hire).
• Valid CA driver’s license, car insurance, and able to drive agency vehicles or as permitted.
• Able to complete background checks including Live Scan and CCL documents.

Responsibilities

• Assessment of medical, behavioral health, and social needs for clients experiencing homelessness.
• Care planning aligned with client needs, including medication, follow-up appointments, and transportation.
• Coordination with the client’s medical providers, with signed releases.
• Behavioral health and substance use linkage with harm reduction where appropriate.
• Provide confidential, holistic, client-centered, trauma-informed services.
• Document performance targets and outcomes for reporting purposes.
• Maintain accurate client records and notes.
• All training as required by program grants must be completed.
• Complete functional timesheets by assigned due dates in compliance with time allocations.
• Actively participate in reflective supervision sessions.
• Create and maintain documentation of processes and procedures related to job duties.
• Other duties as assigned.
