Responsibilities

• Assist with developing a written plan of services; input case data into applications throughout the process.
• Prepare documents, authorizations, and related administrative tasks in tandem with processing staff.
• Monitor consumer progress during service delivery.
• Collaborate with community partners to help individuals with disabilities achieve suitable employment.
• Document service delivery and case progress in the case management system.
