Job summary

South Middlesex Opportunity Council (SMOC) is seeking a Case Manager to provide supportive services for chronically homeless individuals. You will conduct needs assessments, develop service plans, and connect clients to community resources while maintaining regular client contact and confidentiality.

The role requires the ability to drive, travel within the state, and potential hybrid work arrangements. A strong background in case management and substance abuse is essential.

Qualifications

• Bachelor’s Degree requirement or 5+ years working with the target population.
• Substance abuse experience is required.
• Excellent interpersonal and organizational skills.
• Previous experience with group facilitation is preferred.

Responsibilities

• Ensure adherence to policies and protocols for programs serving chronically homeless individuals.
• Develop regular program schedule including activities, meetings, meals, etc.
• Conduct initial service needs assessment with all clients entering the program.
• Develop Individual Service Plan within three days of entry.
• Maintain daily contact with clients and regular contact for at least 24 months.
• Maintain confidentiality and comply with funder requirements.
