This hybrid role connects sober living with PHP/IOP services, coordinating care, transportation, referrals, and documentation to ensure smooth client progress.

Ruby's Place dba Restorative Pathways (RP) is an innovative nonprofit committed to ending domestic violence, human trafficking, and violent crime through hope, advocacy and connection. Our services include emergency shelter, transitional housing, rental subsidies, mental health services, substance recovery, and wrap-around services. All our services are trauma-sensitive and client-centered and are provided at no cost to our clients. All programs are low barrier.

What qualifications are required for this position?

Required:

• High school diploma or equivalent (GED) required.
• Ability to pass all required background screening prior to hire, including criminal background check and DOJ/FBI fingerprinting, per RP and California DHCS requirements.
• Strong organizational skills and the ability to manage and track a caseload of clients with competing deadlines and needs.
• Clear, professional written and verbal communication skills, including comfort documenting in an electronic medical record.
• A calm, non-judgmental, and compassionate approach to working with clients in recovery and their families.
• Ability to work collaboratively across residential, clinical, and medical teams.
• Valid California driver's license and reliable transportation, if assisting with client transportation coordination.
• In possession of a valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles. If a staff member cannot be approved by RP insurance as a driver, employment might not be able to continue. Instances where staff cannot be approved to drive will be reviewed on a case by case basis to determine if employment can continue in their role without driving agency vehicles. Items considered include but are not limited to: the shift the staff member holds and the frequency/necessity of driving during that shift, the amount of time before the staff member may be approved to drive by the insurance carrier, the availability of other staff on site to drive agency vehicles as program needs require, etc.
• Able to complete all required background checks. Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, and ability to meet CCL licensing requirements.

Preferred:

• Bachelor's degree in social work, psychology, human services, or a related field.
• Prior experience in case management, care coordination, or a related support role, ideally in a behavioral health or SUD treatment setting.
• Familiarity with community resources in the Oakland / Bay Area region (housing, employment, benefits, legal aid, healthcare).
• Registered or certified AOD (alcohol and drug) credential, or working toward one — not required for this role, but a plus.
• Bilingual ability, and experience working with culturally and linguistically diverse clients and families.

What You Will Do

Riley Recovery Centers (RRC) is a program within Restorative Pathways. RRC provides safe, compassionate, evidence-based substance use disorder (SUD) treatment for youth and adults. RRC operates a single integrated continuum of care on one property: clients live on-site in a supervised, home-like sober living residence while attending structured clinical programming — Partial Hospitalization Program (PHP, ASAM Level 2.5) and Intensive Outpatient Program (IOP, ASAM Level 2.1) — delivered through individual, group, and family sessions. RRC believes recovery is possible for every person, and our team works together across housing, clinical, and medical services so that treatment feels like one connected experience rather than separate programs under one roof.

RP is hiring a full-time Case Manager within RRC to serve as the connective thread between our sober living residence and our PHP/IOP clinical programs. This is a non-clinical, client-facing coordination role: you will not provide therapy or counseling, but you will be the person who makes sure every client's practical, logistical, and support needs are identified, tracked, and followed through on — from a scheduled appointment or a referral to community services, to helping a client in their final weeks of sober living line up a job and permanent housing. You'll work closely with the Program Manager, AOD Counselors, the House Manager, and RRC's clinical and medical team, and you'll be a steady, organized presence for clients navigating a lot of moving pieces at once.

Responsibilities include

1. Sober Living / Residential Support
   1. Serve as each resident's primary point of contact for case management needs throughout their stay in RRC's sober living residence.
   2. For residents in their free-range period after day 30, check in regularly to confirm they are meeting with you as their case manager, actively working toward employment and permanent housing, and engaging in recovery work outside of scheduled groups.
   3. Coordinate with the House Manager and residential staff on day-to-day needs that affect a client's stability — transportation, documentation, appointments, and other logistics.
   4. Provide backup support for Restorative Pathways housing intake (rules and expectations, welcome packet, facility tour, belongings inventory) when the primary Housing Intake Coordinator is unavailable.
   5. Assist with coordinating client transportation to appointments (e.g., medical visits, the welfare office, medication pickups) alongside the residential and clinical teams as needed.
2. PHP / IOP Outpatient Case Coordination
   1. Own collateral contacts, referrals, and appointment coordination for clients in PHP and IOP — including medical, psychiatric, educational, social services, legal/probation, and community-based supports.
   2. Schedule and confirm client appointments with outside providers and community resources, and follow up to confirm appointments were kept.
   3. Support discharge and transition planning alongside the treatment team, helping ensure referrals, aftercare supports, and follow-up appointments are in place before a client leaves care.
   4. Help coordinate family and support-system involvement in a client's care plan, in partnership with the clinical team and under proper consent.
   5. Flag emerging client needs (housing instability, benefits/insurance issues, transportation barriers, school or legal obligations) to the Program Manager and treatment team promptly.
3. Documentation & Team Coordination
   1. Document all case management activity and collateral contacts in the client's electronic medical record within 24 hours, per RRC's documentation standards.
   2. Participate in the weekly Team & Case Review Meeting, reporting on case management activity, appointment status, and any client needs the clinical team should know about.
   3. Maintain strict client confidentiality in accordance with 42 CFR Part 2 and HIPAA.
   4. Report to the Program Manager on caseload status, outstanding referrals, and any client concerns that need attention.
4. Administrative/Other
   1. Complete functional timesheets by assigned due dates in compliance with time allocations.
   2. Actively participate in reflective supervision sessions.
   3. Create and maintain documentation of processes and procedures related to job duties.
   4. Other duties as assigned.

Working Conditions

This position is eligible for hybrid in person & remote work. Time spent on-site versus remote working will be discussed and agreed upon with your supervisor. You are expected to be availableon-site as necessary given business needs. You will be expected to sit, stand, and move around the facility for extended periods, and to occasionally assist with light lifting (e.g., client belongings during intake). There will be regular exposure to sensitive behavioral health situations; ability to remain calm and follow RRC protocol, escalating safety or clinical concerns to the Program Manager or clinical leadership rather than managing them alone is required.

Local travel in the greater Bay Area may be required.

The pay scale for this role is $32 per hour (non-exempt). There is a $1 per hour language premium for Spanish/English fluency.
