Job summary

Indiana Professional Management Group (IPMG) is hiring a Case Manager for a full-time, hybrid role serving Tippecanoe County, Indiana. The position combines remote work from your home office with travel to community-based meetings several times a week.

Applicants must reside within Tippecanoe County and be able to travel countywide; a Bachelor’s degree in human services or related fields is preferred, with related experience acceptable in lieu of a degree.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field is required.
• A Bachelor’s degree in an unrelated field with 1 year direct experience with people with intellectual/developmental disabilities or 2 years with aging population is acceptable.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
