Job summary

Indiana Professional Management Group (IPMG) is seeking a full-time Case Manager to serve Henry County, Indiana. The role blends remote work from a home office with travel for community-based meetings several times per week; residents must live within Henry County and be able to travel across the county as needed.

Qualifications

• Bachelor’s degree in human services required or related field.
• For bachelor’s in unrelated field, minimum 1 year direct experience with individuals with disabilities or 2 years with aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor’s degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
