Job summary

Indiana Professional Management Group, Inc. is seeking a full-time Case Manager in a hybrid (remote/home office with in-person meetings) role serving Hancock County, Indiana.

Candidates must reside in Hancock County and be able to travel within the county and surrounding counties for community-based meetings. Responsibilities include educating individuals about Medicaid waivers, creating individualized plans, helping access services, and coordinating regular team meetings with providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the Individuals served.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field + 1 year direct experience with individuals with intellectual/developmental disabilities or 2 years aging population experience
