Job summary

Indiana Professional Management Group, Inc is seeking a Case Manager to support Huntington County in a hybrid role. You will educate individuals about Medicaid waivers, develop tailored support plans, assist access to services, and lead community-based meetings several times weekly.

Requirements include a Bachelor's degree in a human services field (or related experience), valid driver's license, and the ability to travel within the county.

Qualifications

• Bachelor's degree in a human services field or related experience.
• Valid driver's license, car insurance, and reliable transportation.
• Able to pass thorough background screens.
• Home office with computer, internet service, and smartphone.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual support plan based on the individuals' strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plans of care with providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or related field

Bachelor's degree in unrelated field with 1 year direct experience with people with IDs or 2 years with aging population
