Job summary

IPMG is seeking a Case Manager for a full-time, hybrid role serving Greene County, Indiana. The position requires remote work from home with travel for community meetings several times a week, and residency within Greene County with reliable transportation.

You will educate individuals about Medicaid waivers, develop person-centered support plans, help access services, and coordinate with providers to review plans of care in team meetings.

Qualifications

• Degree in a human services field or related field as listed above.
• Valid driver’s license, car insurance, and reliable transportation.
• Ability to pass background screens.

Responsibilities

• Educate individuals about the Medicaid waiver process.
• Create individualized support plans based on needs and goals.
• Help individuals access needed services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans of care.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or another human services field

Bachelor's degree in an unrelated field and 1 year direct experience with people with intellectual/developmental disabilities or 2 years experience with aging population
