Job summary

Indiana Professional Management Group, Inc. (IPMG) is recruiting a Case Manager for a full-time hybrid role serving Wayne County, Indiana. You will educate individuals about Medicaid waivers, develop an Individual Support Plan based on strengths and needs, and assist access to required services.

Travel to community meetings is expected, with in-person sessions; residence within Wayne County is required. Collaborating with a care team, you’ll review plans, coordinate services, and participate in

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related field; or 1 year direct experience with individuals with intellectual/developmental disabilities, or 2 years experience with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
