Job summary

Indiana Professional Management Group (IPMG) is hiring for a Case Manager in a full-time hybrid role. The position requires residing in Huntington County, Indiana with travel across the county and nearby areas for in-person meetings.

You will educate clients on Medicaid waivers, develop individualized support plans, help access needed services, and lead community-based team meetings with providers. Requires a human services bachelor’s degree or equivalent experience and valid driving credentials.

Qualifications

• Bachelor's degree in a human services field or related experience is required.
• Valid driver’s license, car insurance, and reliable transportation are required.
• Ability to pass thorough background screens and maintain a designated home office setup with internet and smartphone.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual support plan based on the client’s strengths and goals.
• Help individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings to review care plans and coordinate with providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field with 1+ year experience serving people with intellectual/developmental disabilities OR 2+ years experience with aging population
