The role involves developing case plans and collaborating with a multidisciplinary team to support housing, legal, medical, and employment goals.

The position requires a Bachelor's degree in human services or related field, social work or professional counseling licensure, and experience with at-risk populations.

Qualifications

• Bachelor's degree in human services or related field is required.
• Social work licensure in the appropriate jurisdiction is required.
• Professional counseling licensure in appropriate jurisdiction is required.
• One-year experience with at-risk clients in direct service.

Responsibilities

• Interview clients for direct service needs and develop case plans.
• Reevaluate goals with clients and conduct home/community visits on the entire caseload.
• Create and maintain case records and progress notes.
• Collaborate with multidisciplinary teams including legal, medical, and housing professionals.
• Provide referrals for emotional, legal, medical, housing, and employment needs.
