Job summary

Pierce County in Washington is seeking a seasoned case manager to support older and disabled adults in Medicaid services. You will assess needs, authorize and oversee care plans, and monitor progress while working from home after an initial in-office period.

You will join the Aging and Disability Resources division within the Human Services department, contributing to a compassionate, client-centered team. This grant-funded role includes a 10% field work pay and a hybrid work arrangement.

Qualifications

• Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology, or related field.
• Two years or more experience providing direct human services, preferably in the area of providing services to senior citizens is required.
• Masters degree in behavioral health sciences AND
• One year of experience in the social services field preferred.

Responsibilities

• Responsible for assessment, coordination of services, and advocacy of older and disabled adult clients.
• Provide comprehensive assessment to determine any individualized client needs.
• Provide formal reassessment at specific intervals to gauge progress and continuing needs.
• Provide care planning which requires decisions about how the identified needs can be met and make appropriate referrals.
• Respond to individuals seeking information and assistance by providing routine information, referral, and assistance; direct individuals seeking appropriate crisis interventions.
