Job summary

Bluestone Physician Services in Minnesota is seeking a Care Coordinator to support seniors in navigating healthcare, accessing waiver services, and coordinating with DHS and CMS requirements.

The role is hybrid, with a mix of in-home visits and remote work, primarily serving Maplewood and St. Paul areas. Full-time schedule 8-5 M-Thu and 8-3 on Fri; 65k-75k USD, commensurate with experience.

Qualifications

• Current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW) - New grads encouraged to apply!
• Minnesota Registered Nurse license
• One or more years of experience with geriatric or certified disabled populations in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU or Assisted Living settings preferred
• Must have a valid driver’s license

Responsibilities

• Manage member caseloads within assigned geographic area.
• Coordinate face-to-face visits and elder waiver oversight.
• Conduct annual PCA assessments and develop living tools for assisted living residents.
• Review health needs, set goals, and create individualized care plans.
• Connect members with community and state resources and services.
• Support safe transitions of care and collaborate with care teams for discharge follow-up.

Education

Current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW)

Minnesota Registered Nurse license

Geriatric/care coordination experience
