Bluestone Physician Services is seeking a Care Coordinator in Minnesota to empower seniors to navigate their healthcare journey. You will manage DHS and CMS requirements for MSHO/MSC+ members while coordinating care and resources with families and care teams.

The role is hybrid, combining home visits and remote work, with standard daytime hours and no evenings or weekends. Competitive salary and benefits are offered.

Qualifications

• Minnesota Registered Nurse license or MN Licensed Social Worker (LSW/LMSW/LISW/LICSW) is required or encouraged for new grads.
• 1+ year of experience with geriatric or disabled populations in case management or care coordination preferred.
• Valid driver's license is required.

Responsibilities

• Coordinate face-to-face visits within the member's geographic area.
• Manage Elderly Waiver and related community services.
• Conduct annual PCA assessments and develop individualized care plans.
• Review health needs and set goals aligned with member priorities.
