Job summary

Healthkeeperz is seeking a CAP Case Manager to provide essential case management for the CAP program. The role includes assessments, eligibility determinations, and care planning to coordinate medical, social, and financial services for disabled adults and children in their communities.

The position follows a hybrid model with regular in-person home visits and mileage reimbursement. Starting pay ranges hourly, with full-time benefits including insurance, PTO, and retirement options.

Qualifications

• Bachelor’s degree in social work with related community experience or nursing-related background within 90 days NC Medicaid training.
• Two+ years of relevant community experience in health/medical field preferred for non-social-work degrees.
• Must have experience with home care, long-term care, or personal care settings where CAP applies.
• NC Medicaid‑certified training within 90 days of employment is required or trainee pathway available.

Responsibilities

• Conduct initial screenings and assessments of patients and families.
• Identify social service needs and connect patients to resources.
• Evaluate eligibility, needs, and support systems.
• Develop and update individualized care plans; educate on options and referrals.
