Job summary

NAMI San Diego & Imperial Counties is seeking a Family Support Navigator to provide case management and system navigation for adults connected to the Coordinated Family Supports program in Imperial County. Hybrid work from home and in-person in Imperial Valley communities are involved.

You will support clients and families to access services, overcome barriers, and pursue goals while maintaining confidentiality and culturally appropriate services.

Qualifications

• Bilingual in English and Spanish.
• Associates degree in a human services field OR at least 3 years of experience supporting individuals with IDD; Bachelor's degree preferred.
• At least one year of experience supporting individuals living with IDD.
• In-depth knowledge of Imperial County's services, resources, and culture.
• Excellent planning, organizing, problem-solving, time management, and coordinating skills.

Responsibilities

• Maintain and manage an active caseload.
• Complete assessments to determine client needs and develop written support plans.
• Maintain regular contact with clients.
• Connect families with resources and assist with benefits applications.
• Document services, intakes, and visits within three business days.
• Attend supervision sessions to review progress and documentation.
• Collaborate with San Diego Regional Center staff, clients’ families, and other providers.
• Participate in team meetings and community outreach activities.
• Build relationships with clients, families, and providers; give presentations as needed.
• Assist with reports and invoicing for the program.
