Job summary

Qualifications

• Associate’s degree in a related Human Services discipline and two years of related experience.
• Strong interpersonal and communication skills.
• Ability to document in Epic EHR and use Microsoft Word/Excel/Outlook.
• HIPAA compliance and patient confidentiality.

Responsibilities

• Serve as a family system navigator to support I/DD and MH navigation needs.
• Provide emotional, informational, and instrumental support to families.
• Coordinate with UNC pediatric providers and partner agencies to connect services.
• Document work in Epic and maintain accurate records in Word/Excel/Outlook.
• Attend regular team meetings and participate in ongoing FN training materials.
