The Arc Southern Chesapeake is hiring a Supports and Services Specialist to lead admissions, referrals, and transition processes across Calvert, Charles, St. Mary’s and Anne Arundel counties.

The role emphasizes person-centered planning using CQL’s Personal Outcome Measures to set individualized goals and improve service delivery. Qualifications

• Bachelor’s Degree or higher in Human Services or related field.
• Minimum of three years relevant experience preferred.
• Knowledge of Maryland DDA service delivery systems and regulatory requirements.
• Working knowledge of Person-Centered Practices (CQL’s POM preferred).

Responsabilités

• Manage admissions from referral through placement ensuring funding reflects needs.
• Facilitate communication between stakeholders regarding application status.
• Maintain admissions documentation and tracking systems.
• Prepare monthly reports and monitor metrics for process improvements.
• Develop PCP with individuals and planning discussions to promote collaboration.
