Job summary

The Arc Of Southern Maryland is seeking a Supports and Services Specialist to lead admissions, referrals, and transitions in a hybrid role across the Annapolis area and southern Chesapeake region.

You will help develop Person-Centered Plans (PCP) using CQL�s POM, engage stakeholders, and monitor outcomes to improve service delivery while ensuring client goals and preferences are respected.

Qualifications

• Bachelor’s Degree or higher in Human Services, or other related degree required
• Minimum of three years relevant experience preferred
• Demonstrated knowledge of Maryland DDA service delivery systems and regulations
• Working knowledge of Person-Centered Practices (CQL’s POM preferred)
• Knowledge of IDD and common support strategies
• Healthcare knowledge is preferred

Responsibilities

• Lead admissions from referral through placement with appropriate funding and needs alignment
• Coordinate intake, maintain documentation, and track metrics for process improvements
• Develop and implement PCP goals using CQL’s POM with the team and individuals served
• Support planning meetings, data collection, and cross-team collaboration
• Provide post-admission follow-up to ensure service fit and continuity
