Job summary

The Arc Southern Chesapeake seeks a Supports and Services Specialist in Annapolis, MD to lead admissions, referrals, and transitions. You will promote person-centered planning and ensure services align with funding and eligibility, while engaging with families and stakeholders.

The role is hybrid, servicing Calvert, Charles, St. Mary’s and Anne Arundel regions. Requires a Bachelor’s in Human Services and 3+ years of related experience; DDA knowledge preferred.

Qualifications

• Bachelor's Degree or higher in Human Services or related field is required.
• Minimum of three years of relevant experience preferred.
• Knowledge of Maryland DDA service delivery systems and regulatory requirements.
• Working knowledge of Person-Centered Practices (CQL’s POM preferred).
• Knowledge of IDD and common support strategies; healthcare knowledge is a plus.

Responsibilities

• Manage admissions from referral through placement, ensuring funding and eligibility.
• Coordinate transitions and maintain documentation and timelines for admissions.
• Develop and maintain PCP quality improvement using CQL’s POM; track outcomes.
• Collaborate with individuals served and teams to promote person-centered planning.
