Protects and promotes the health of Richmond City residents by supporting chronic disease prevention initiatives that improve access to health education, community resources, and evidence-based prevention strategies. This is accomplished by planning, coordinating, and implementing chronic disease prevention programs; conducting community outreach and health education; developing and maintaining partnerships with community organizations, healthcare providers, and other stakeholders; connecting individuals to available health resources; and supporting community events and public health initiatives.
Provides support for the Richmond and Henrico Health District’s Community Health Improvement Plan (CHIP) by advancing chronic disease priorities through program planning, implementation, evaluation, and continuous quality improvement. Assists with identifying community needs, monitoring program activities and outcomes, maintaining program records and reports, and ensuring activities align with applicable federal, state, and local public health guidance, grant requirements, and agency priorities.
Serves as a resource on chronic disease prevention by providing education, technical assistance, and outreach to community partners, organizations, and residents. Collaborates with internal and external stakeholders to strengthen partnerships, increase community engagement, and promote sustainable strategies that address health disparities and improve health outcomes. Provides support to the Health District and participates in additional public health initiatives, emergency response activities, and other duties as assigned to support the agency’s mission.
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHSIG Exclusion List check, employment reference check and E-Verify.
