Qualifications

Responsibilities

• Coordinate and facilitate ISP development for individuals on caseload.
• Monitor service provision by DDS funded providers.
• Maintain regular contact with individuals, families and providers.
• Represent the Area Office and build collaborative partnerships.
• Guide transitions from youth to adult services and coordinate related supports.
