Job summary

Qualifications

• At least three years of relevant professional human services or social work experience.
• Experience with ISP or service coordination is preferred.

Responsibilities

• Coordinate and facilitate ISP development for individuals on assigned caseload.
• Monitor and ensure DDS-funded and community supports are provided.
• Maintain communication with individuals, families, providers, and agencies.
• Serve as a liaison to partner with state agencies and advocate for clients.
• Provide crisis intervention and arrange follow-up planning as needed.
