Job summary

TURN is seeking a Housing Specialist in Fresno, CA, to provide case management and housing linkages for clients in need of housing and related services.

The role involves coordinating referrals, advocating for whole-person care, and maintaining thorough documentation while protecting client confidentiality. Strong MS Office and EHR skills plus relevant certifications or degrees are preferred.

Qualifications

• Master's degree or bachelor's degree with four years of experience in social services.
• AA degree in social service or related field or High School Diploma with 5 years’ experience.
• SUD CM certification from recognized board is preferred and registration required.
• Valid CA driver’s license and current auto insurance per contract requirements.
• Experience providing mental health and/or substance use disorder services preferred.
• Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Other relevant duties as assigned.
• Specialty in housing related duties and housing linkages.
• Facilitate maintenance requests for clients who live on site.
• Participate in weekly housing inspections, to ensure clients are maintaining units in habitable conditions.
