Job summary

TURN in Hanford, CA is seeking a Case Manager to coordinate medical, educational, social, vocational, housing referrals and advocate for clients, working toward whole-person care.

The role requires a Bachelor’s degree in social services with at least one year of experience; a CA driver’s license; and willingness to provide field-based services, documentation, and occasional on-call coverage.

Qualifications

• Bachelor’s degree in social services.
• For SUD CM a certification from recognized board is preferred and registration required.
• Housing: two years of experience and an AA degree in social service or related field or High School Diploma and 5 years’ experience.
• Valid CA driver’s license and current auto insurance per contract requirements.

Responsibilities

• Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
• Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
• Facilitate placement including pre-placement visits, and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client’s care.
• Participate in scheduled program meetings.
• Assist in the screening process and ensure the accuracy of data collection.
• Facilitates coordinated referral process for mental health services.
• Provides onsite and school-based services to youth enrolled in the MHSSA Program.
• Assist with community trainings and presentations as needed.
• Other relevant duties as assigned.
