Qualifications

• Experience working with individuals experiencing chronic homelessness.
• Familiarity with HMIS and housing resources.
• Strong interpersonal communication and advocacy skills.
• Ability to manage caseloads and maintain confidentiality.
• Valid Oklahoma Driver's License preferred.

Responsibilities

• Manage a caseload up to 20 households.
• Coordinate with encampment teams and housing providers.
• Assist with housing applications and move-ins.
• Provide weekly in-person case management for one year post-housing.
• Document client interactions in HMIS.
• Monitor progress and adjust service plans.
