Job summary

Qualifications

• Associate's degree in human services or related field preferred
• Personal vehicle with current automobile insurance and valid Maryland driver’s license required

Responsibilities

• Identify, secure, and retain housing placements.
• Assist with the inspection process to ensure safety standards.
• Provide housing-based counseling on tenant rights and responsibilities.
• Develop and implement individualized housing plans.
• Promote plan implementation with information and referrals to resources; address tenancy issues.
• Build landlord relationships and mediate to expand housing options.
• Prepare and maintain client records for leasing and grant compliance.
• Complete HMIS documentation and data collection for care coordination.
• Document in the client’s electronic health record clearly and timely.
