Qualifikationen

• Bachelor's degree in Human Services or related field is required.
• Minimum 6 months to 1 year housing or case management experience.
• Strong organizational skills are essential.
• Excellent communication skills required.
• Proficiency with Excel for spreadsheets and data management.
• Ability to read, write, and present information clearly.
• Bilingual skills are a plus.

Aufgaben

• Provide housing stabilization services and maintain participant records.
• Open Housing Episodes in Epic and ensure medical records are established.
• Coordinate referrals for supportive services and track engagement.
• Document services with quick notes, intakes, and treatment plans.
• Manage data in group folders and track target measures.
• Interact with participants and external agencies professionally.
