Job summary

Yakima Neighborhood Health Services is seeking a dedicated professional to provide case management and outreach to homeless individuals, helping access housing, entitlements, and supportive services. The role emphasizes collaboration with partner agencies and maintaining confidential client records.

The ideal candidate has an associate degree in health or behavioral sciences and experience in social services, with bilingual Spanish/English desirable.

Qualifications

• Associate degree in behavioral or health sciences or related field; two years related work experience.
• Knowledge of case management, government benefits and community resources; experience in social service delivery.
• Bilingual Spanish/English desirable; strong written and spoken communication.

Responsibilities

• Identify and outreach to homeless persons to determine who needs services.
• Provide case management support including home visits.
• Assess client needs and strengths to develop service plans.
• Coordinate with agencies to stabilize housing and self-sufficiency.
• Assist with accessing entitlements and completing forms.
• Maintain confidentiality of client interactions and medical records.
• Represent YNHS in local meetings as requested.
