Job summary

Yakima Neighborhood Health Services in Yakima, WA seeks individuals to support outreach to homeless persons and provide hands-on case management. You will assess needs, develop service plans, and coordinate with partner agencies to improve housing stability.

The ideal candidate has an associate degree in behavioral or health sciences, bilingual English/Spanish preferred, and 2+ years of related experience. Valid driver’s license and insurance required; background check will be conducted.

Qualifications

Associate degree in behavioral or health sciences or related field.
Bilingual in English/Spanish preferred.
Two years of closely related work experience.
Knowledge of effective case management practices.
Knowledge of government benefits and community resources.
Experience in effective social service delivery.
Knowledge of addiction and mental health pathologies.
Must pass background check.
Must have valid driver’s license and car insurance.

Responsibilities

Identify and outreach to homeless persons to determine who is in need of services.
Provide case management support, including scheduled and unscheduled home visits.
Assess client needs to determine limitations and strengths.
Coordinate with agencies to stabilize housing and improve self-sufficiency.
Develop service plans with clients to set goals and action steps toward self-sufficiency.
Assist in securing food, shelter, clothing and referrals to supportive services.
Assist in accessing entitlements.
