Job summary

Yakima Neighborhood Health Services is seeking a compassionate professional to work with individuals who are homeless or in transitional housing. The role focuses on outreach, case management, and coordinating access to community resources.

The ideal candidate will have an associate degree in health/science fields, bilingual English/Spanish preferred, and experience in social services. A driver’s license and reliable vehicle are required to support community outreach efforts.

Qualifications

• Associate degree in behavioral or health sciences or related field.
• Two years of related work experience.
• Knowledge of effective case management practices.
• Knowledge of government benefits and community resources.
• Experience in effective social service delivery.
• Knowledge of addiction and mental health pathologies.
• Ability to pass a background check.
• Must have valid driver's license and car insurance.

Responsibilities

• Identify and outreach to homeless persons to determine who is in need of services.
• Provide case management support, including scheduled and unscheduled home visits.
• Assess client needs to determine limitations and strengths.
• Coordinate with agencies to stabilize housing and improve self-sufficiency.
• Develop service plans with clients with goals and action steps toward self-sufficiency.
• Assist securing food, shelter, clothing and referrals to services.
• Assist in accessing entitlements.
