Job summary

Yakima Neighborhood Health Services is seeking a qualified candidate to support homeless outreach and provide comprehensive case management. You will assess client needs, develop service plans, and coordinate with community partners to enhance housing stability and access to benefits.

The role requires an Associate degree or equivalent experience, strong communication skills, and the ability to work with diverse populations.

Qualifications

  • Associate degree in behavioral or health sciences or related field.
  • Two years closely related work experience; education may substitute year for year.
  • Knowledge of case management and government benefits and resources.
  • Experience in social service delivery and knowledge of addiction/mental health pathologies.
  • Proficiency with computer operating systems.
  • Bilingual Spanish/English desirable.
  • Background check and compliance required.

Responsibilities

  • Identification and outreach to homeless persons to determine who is in need of services.
  • Provide case management support, including scheduled and unscheduled home visits.
  • Assess needs to determine client limitations and strengths.
  • Work with agencies to maximize clients’ housing stability and self-sufficiency.
  • Develop service plans with clients and set goals towards self-sufficiency.
  • Assist with securing food, shelter, clothing and referrals to services.
  • Assist in accessing entitlements and completing forms.
  • Crisis intervention and client advocacy.
  • Maintain confidentiality of client interactions and records.
  • Participate in staff meetings and quality assurance activities.
  • Represent YNHS in local meetings as requested.

Our Ideal Candidate
  • Associate degree in behavioral or health sciences or related field and two years closely related work experience.
  • Qualifying work experience may be substituted year for year for education.
  • Knowledge of effective case management practices; knowledge of government benefits and community resources; experience in effective social service delivery.
  • Knowledge of addiction and mental health pathologies.

Day To Day

  • Work with agencies/organizations to maximize clients’ abilities to stabilize housing and improve self-sufficiency.
  • Develop service plan with clients to set goals (including self- management goals) and action steps toward self-sufficiency.
  • Provide assistance with securing food, shelter, clothing and referrals to other supportive services.
  • Provide assistance in accessing entitlements and completing forms and applications.
  • Crisis intervention.
  • Advocate on behalf of clients.
  • Maintaining confidentiality of client interactions and medical records.
  • Participate in staff in-service meetings.
  • Participate in quality assurance activities and peer review as requested.
  • Maintain organized, clean, efficient and confidential work area.
  • Represent YNHS in local meetings as requested.
  • Perform related work and other tasks as requested.
