Job summary

Father Bill’s & MainSpring in Brockton, MA is seeking a Stabilization Case Manager for a full-time, hourly position. You will support families moving from homelessness to housing, coordinating referrals and maintaining permanent housing.

The role requires a Bachelor’s degree or equivalent work experience; CPR certification is required and provided by FBMS. AED and First Aid may also be required and provided.

Qualifications

• Bachelor's degree or equivalent work experience.
• CPR certification is required and provided by FBMS; AED and First Aid may also be required and provided by FBMS.

Responsibilities

• Complete intake process within required time frame for all family members 18+, housing first framework.
• Collect and maintain data-based statistics, maintain accurate case records including data entry/ETO monthly and quarterly.
• Meet with participants at least quarterly to monitor progress and implement individual service plans monthly.
• Set up and maintain accurate case files and adhere to funder requirements.
• Provide outreach and assessment in the community as needed.
• Collaborate, refer and case conference with shelter, housing search, child services, workforce development and Prevention/Diversion staff including TPP.
• Complete home visits and assist participants to access community resources providing support and transportation.
• Participate in supervision, agency and community meetings and trainings when required.
• Initiate crisis intervention when necessary.
• Demonstrate empathy, professionalism, communication, record-keeping and reliability.
• Adhere to safety policies and maintain work ethic and initiative.
