Job summary

The Night Ministry in Chicago seeks a Case Management Program Manager to unify adult and youth outreach case management into one coordinated continuum. You will own housing retention strategies and supervise a team focused on housing outcomes, cross-program collaboration, and service quality.

Responsible for standardized protocols, internship programs, and shared referral pathways with partner programs to ensure seamless transitions for clients across services.

• Experience leading housing-focused case management programs.
• Strong leadership and people-management skills.
• Ability to coordinate across multiple teams and programs.

Responsibilities

• Housing stabilization design and implementation with outcomes metrics.
• Carry a defined retention caseload within role time allocation.
• Own outreach case management outcomes across adult and youth teams.
• Identify delivery gaps and drive strategy improvements.
• Develop and maintain case management protocols and standards.
• Coordinate Medical Respite case management as it develops.
• Supervise the Case Manager Supervisor for Adult Services and two youth managers.
• Support day-to-day supervision, case consultation, and caseload management.
• Lead internship program for case managers with curriculum and evaluation.
• Develop shared referral pathways and handoff processes with program leaders.
