Job summary

AMI Housing is seeking a Service Coordinator in Grass Valley, CA to provide high-quality case management for residents transitioning from homelessness to permanent housing. You will be the central contact for participants and work with the Director of Supportive Housing to coordinate services.

You will conduct intake visits, maintain caseloads, link to health, behavioral health, education, and vocational resources, and ensure accurate documentation in HMIS and other systems.

Qualifications

• Bachelor’s degree in a social services field required.
• Minimum 2 years of case management or housing experience.
• Bilingual English/Spanish preferred.
• Successful background checks and TB clearance.

Responsibilities

• Provide weekly case management and tenant support.
• Develop individualized service plans and link to services.
• Maintain accurate case notes in HMIS/related systems.
• Assist with housing applications and documentation.
