Job summary

AMI Housing, Inc. in Grass Valley, CA is hiring a Service Coordinator to provide high-quality case management for residents transitioning from homelessness to tenancy.

You will work with a team under the Director of Supportive Housing to coordinate housing and supportive services, conduct assessments, and link clients to health, education, and job training resources. The role requires a Bachelor’s degree in a social services field and prefers bilingual English/Spanish.

Qualifications

• Knowledge of Housing First principles and tenant rights and responsibilities.
• Ability to assess participant needs, develop service plans, set goals, monitor progress and coordinate services.
• Ability to advocate for participants, resolve problems, and maintain professional boundaries.

Responsibilities

• Evaluate individuals for permanent housing with the team and Director of Supportive Housing.
• Develop supportive relationships and maintain an assigned caseload of residents.
• Prepare forms and documentation for housing certification and recertifications.
• Conduct initial visits and ongoing weekly case management sessions to monitor progress.
• Provide referrals and linkages to health, mental health, education, and vocational training.
• Maintain accurate program notes and participate in time studies and data systems (HMIS, MHSA, EHR).
• Assist with Medi-Cal information, time studies, and outreach aligned to Medi-Cal objectives.
• Coordinate with partners to expand Medi-Cal awareness and outreach initiatives.
