Job summary

YouthLink is seeking a Case Manager – Scattered Site Housing to support youth ages 18-24 in crisis, helping them access housing and wraparound services. You will coordinate referrals for housing, counseling, financial counseling, job readiness, and higher education, while engaging with community partners to expand services.

The role emphasizes collaboration, timeliness, and a holistic approach to stabilize clients and promote self-sufficiency in a Minnesota setting.

Qualifications

• 1+ years of case management experience.
• 2 years’ experience working with at-risk young adults.
• 2 years’ experience working with homeless youth.
• 2 years’ experience with community engagement.
• Must be able to come to work on time for regularly scheduled shifts.
• Valid MN State Driver’s License and insurance.

Responsibilities

• Provide wraparound case management and coordination for youth ages 18–24 in crisis.
• Assist clients in obtaining housing stability and self-sufficiency services.
• Engage with community partners to expand available services.
• Refer clients to housing, counseling, financial counseling, job readiness programs, and higher education.
