Job summary

Southwest Network Team seeks a housing specialist as part of the Assertive Community Treatment (ACT) team in a community setting. You will assess housing and daily living needs, lead in service planning, and coordinate direct services to members to support independent living.

Responsibilities include ISP development, after-hours coverage, and on-call support. A relevant degree and housing experience with ACT teams are preferred.

Qualifications

• Master’s or Bachelor’s degree in behavioral health or related field.
• Bachelor’s degree in a non-related field and 1 year related experience.
• Associate degree and 1 year related experience.
• High School diploma or equivalent and 3 years related experience.

Responsibilities

• Assess housing, residential and daily living skills needs and plan services.
• Coordinate, monitor and provide direct services in a community setting.
• Develop ISPs and coordinate services related to ISPs.
• Provide after-hours care and on-call coverage for ACT team members.

Education

Master’s or Bachelor’s degree in behavioral health or related field
