Job summary

Avalon Housing in Ann Arbor, MI is seeking a full-time Hickory Way Support Coordinator to provide direct case management and coordinated services for tenants, leveraging a Housing First approach. The role emphasizes outreach, assessments, and collaboration with community resources to address housing and psychosocial needs.

The position requires experience with homelessness, crisis response, and strong communication skills.

Qualifications

• Bachelor's or Master's degree in a social or behavioral science or three years of relevant experience.
• Experience working with people who are homeless or facing mental health/substance use challenges.
• Skilled in crisis response situations.
• Strong verbal and written communication skills.
• Interest and experience in engaging difficult clients with harm reduction.
• Commitment to racial equity and serving diverse populations.

Responsibilities

• Provide direct case management or coordinate with case managers to ensure ongoing outreach.
• Conduct tenant assessments and develop action plans to meet goals.
• Collaborate with Property Management to prevent evictions.
• Provide crisis intervention as needed.
• Assist tenants with accessing medical, mental health, and psychosocial supports.
• Support tenants in budgeting and basic life skills.
• Link tenants to employment and skill development opportunities.
• Foster community and peer support among tenants.
• Assist with activities of daily living and transportation for appointments.
• Maintain required program documentation and progress notes.
