Job summary

Elica Health Centers is expanding its Community Supports program in California. The Housing Navigator will link Medi-Cal clients experiencing homelessness to housing resources, develop individualized housing plans, and coordinate with landlords and agencies to secure stable housing and services.

Role emphasizes collaboration with ECM, CS team, and health plans to facilitate move-ins, manage HMIS data, and support clients through barriers to sustainable housing, while maintaining high-quality

Qualifications

• Minimum of High School Diploma; Associate or Bachelor’s preferred.
• Bilingual Spanish, Farsi or Russian is highly preferred.
• Strong written, verbal and interpersonal communication is essential.
• Experience with housing navigation or case management in related fields is preferred.

Responsibilities

• Link eligible Medi-Cal clients experiencing homelessness to housing resources.
• Develop housing plans to achieve permanent housing and stability.
• Coordinate with landlords, agencies, and health plans for services and deposits.
• Maintain HMIS data and complete Needs Assessments and Stabilization plans.
• Provide client advocacy and connect to community resources and support services.
