Job summary

Simple Solutions Psychotherapy in California is seeking a Community Supports Case Manager – Housing Navigator to assist Medi-Cal members experiencing homelessness or at risk of homelessness in securing and maintaining permanent housing. You will liaise with landlords, housing authorities, and care teams to reduce barriers and promote stability.

The role covers housing navigation, move-in support, tenancy sustainment, financial literacy, and care coordination.

Qualifications

• Bachelor’s degree in a related field preferred.
• Minimum of two years of housing navigation or homeless services experience.
• Valid California driver’s license and reliable transportation.
• Experience with Medi-Cal, CalAIM, Community Supports, or Enhanced Care Management preferred.

Responsibilities

• Conduct housing needs assessments and identify barriers to permanent housing.
• Develop individualized housing plans to support each member’s goals.
• Identify and secure affordable housing opportunities meeting member needs.
• Coordinate with landlords, property management, and community partners.
• Coordinate move-in and tenancy processes, including paperwork and deposits.
