Job summary

Boston Medical Center’s Living Well at Home Program seeks a Housing & Community Support Specialist to provide housing-focused case management for high-risk patients experiencing homelessness in a congregate setting.

You will engage patients, coordinate with housing providers and care teams, develop individualized service plans, and connect clients with community resources to stabilize tenancy and improve health outcomes.

Qualifications

• Bachelor’s degree in a behavioral health or related field OR two years of relevant work experience OR lived experience of homelessness or behavioral health conditions
• Minimum of 2 years prior healthcare, public health or social services work in community-based setting
• Fluency in Haitian Creole or Spanish preferable

Responsibilities

• Engage patients and enroll into services; provide advocacy and case management
• Coordinate housing plans with interdisciplinary teams
• Liaise with landlords and property managers throughout the housing process
• Monitor progress and update individualized service plans as needed
