Responsibilities

• Provides follow-up housing placement and stabilization services in home or office settings, assists with rent/utilities, and connects clients to community resources.
• Engages all clients using a housing-first approach.
• Coordinates community integration and life skills training for independent housing success.
• Hosts on-site housing search assistance with housing agencies (e.g., BHA, HomeStart).
• Completes intakes and assessments to determine needs and eligibility for services and benefits; provides case management and crisis counseling.
• Transports clients to housing appointments and maintains service plan records.
• Maintains up-to-date contacts and progress notes for the department.
• Works in a multidisciplinary team with shelter social services, work rehab, and health services.
