Job summary

Boston Medical Center’s Living Well at Home program seeks a Senior Housing & Community Support Specialist to provide housing-focused case management for high-risk patients experiencing homelessness. You will own special departmental projects, mentor staff, and collaborate with housing providers and care teams to secure housing opportunities.

You will engage patients in goal setting, develop individualized service plans, and coordinate access to social services, health care, and housing

Qualifications

• Bachelor’s degree in behavioral health or related field or two years of relevant work experience, or lived experience of homelessness/behavioral health conditions.
• Experience in healthcare, public health, or social services in community-based settings.
• Fluency in English; bilingual in Haitian Creole or Spanish is preferable.

Responsibilities

• Visits and supports patients through intensive in-home and community-based outreach.
• Builds rapport and trust with patients using patient-centered approaches.
• Initiates face-to-face contact and screening with eligible patients.
• Guides housing plans and uses motivational interviewing to help patients achieve goals.
• Coordinates with care teams and housing providers to address barriers to housing.
