Job summary

Jericho Project in New York City seeks a Life Coach to provide case management and service coordination at the Melrose 78-unit Permanent Supportive Housing program. The role requires experience with adults facing mental illness and substance use histories, with weekly to monthly contact with colleagues.

The Life Coach will conduct assessments, develop care plans every six months, and coordinate with psychiatrists and other providers.

Qualifications

  • Bachelor's degree in human services or related field.
  • Experience with homelessness, substance use, mental health or trauma settings preferred.
  • Willing to work a weekend day.
  • Strong organizational, writing, communication, and computer skills.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Maintain a case load of high-risk/high-intensity tenants.
  • Provide enhanced case management using Housing First approaches and evidence-based practices.
  • Conduct assessments and program intakes for residents.
  • Ensure data accuracy and complete documentation.
  • Develop and update person-centered service plans every 6 months.
  • Use trauma-informed and culturally responsive engagement techniques.
  • Identify residents' strengths, needs, and preferences.
  • Liaise with the program psychiatrist and manage appointment times.
  • Provide referrals and linkages to public benefits.
  • Maintain network of resources to support community integration.
  • Lead groups focusing on life skills and mental health counseling.
  • Coordinate with Workforce Opportunities to link residents to employment aid.
  • Coordinate aftercare and post-discharge follow-up.
