Qualifications

Responsibilities

• Develop and monitor individualized service plans to obtain permanent housing.
• Document files and complete HMIS reports accurately and on time.
• Coordinate with health and social service providers and transport to appointments.
• Educate families about tenant rights, budgeting, and community resources.
