Job summary

Eliot Community Human Services, Inc. in Cambridge seeks a Housing First Coordinator to join the Homeless Services Division. You will provide person-centered, strengths-based case-management to adults transitioning to housing, aiming for long-term stability and independence.

Responsibilities include needs assessment, care coordination with partners, facilitating access to benefits, and supporting residents through housing transitions with trauma-informed de-escalation and advocacy.

Qualifications

• Bachelor’s or Master’s degree in a human services field or social work, preferred.
• For candidates with lived experience with substance use disorder who have 2 years sustained sobriety: High school diploma/GED and at least one year of paid or volunteer experience working with adults with mental illness and/or substance abuse.

Responsibilities

• Provide person-centered, strengths-based case-management to residents.
• Conduct initial needs assessment and collaboratively develop a plan with the resident to obtain benefits and resources.
• Collaborate across the staff team to ensure resident perspective is incorporated.
• Support new residents in all phases of transitioning into housing with partnering organizations.
• Assist clients in integrating into communities by linking to resources and supports.
• Provide mentoring, advocacy, coordination, and direct assistance for daily living skills.
• Promote health and wellness, including access to medical care and HIV resources.
• Intervene in crisis situations using trauma-informed, de-escalation techniques.
