Eliot Community Human Services, Inc. in Massachusetts is seeking a Housing First Coordinator to join the Homeless Services CSP-HI.

This role provides person-centered, strengths-based case management to unsheltered adults in transition to housing, with focus on long term housing stability, financial independence, and emotional well-being. You will work onsite Monday through Friday, engage with community resources, advocate for client-centered services, and collaborate with coalitions to address

Qualifications

• Bachelor's degree in a human services field or social work or related field, preferred.
• 2 years case management or comparable experience with the homeless population.
• Must possess a valid Massachusetts driver's license and maintain a personally owned vehicle.

Responsibilities

• Work onsite at the office and in the field with flexibility to respond to needs as they arise throughout the day.
• Engage in client-centered/harm-reduction/Housing First framework.
• Forge and maintain positive working relationships with community members and coalitions.
• Advocate to ensure client interactions with support services are person-centered and recovery oriented.
• Collaboratively develop and implement needs assessments and action plans with clients to improve housing stability.
• Address ongoing clinical and systemic needs with attention to risk factors.
• Develop and implement safety plans as needed.
