Job summary

Eliot Community Human Services, Inc. is seeking a Housing First Coordinator to support unsheltered adults in transition to housing, using a Housing First model and person-centered case management.

Based in Boston, MA, this role emphasizes collaboration with community resources, development of needs assessments and action plans, and ensuring access to long-term housing, financial independence, and emotional well-being within a dedicated Community Support Team.

Qualifications

• Bachelor’s degree in human services or social work or related field.
• 2 years case management or comparable experience with homeless population (or related lived experience).

Responsibilities

• Work onsite at office and in the field with flexibility throughout the day.
• Engage in client-centered/harm-reduction/Housing First framework.
• Forge and maintain positive relationships with community members and coalitions.
• Advocate to ensure client interactions are person-centered and recovery oriented.
• Develop and implement needs assessments and action plans with clients.
• Address ongoing clinical or systemic needs with attention to risk factors.
• Develop and implement safety response plans as needed.
